Effective Date: September 28, 2026

These Terms and Conditions (“Terms”) govern your access to and use of the UDOT SR mobile application, website, and related services (collectively, the “Service”).

The Service was developed and is operated by NIKALogic IT Solutions, Inc. (“NIKALogic,” “we,” “us,” or “our”) on behalf of the Utah Department of Transportation (“UDOT”).

By accessing or using the Service, creating an account, or submitting a roadway service request, you agree to these Terms. If you do not agree, do not use the Service.

1. Purpose of the Service

UDOT SR allows members of the public to submit reports concerning roadway conditions and related transportation issues. Available features may include:

  • Submitting a report as a guest;
  • Creating and managing a registered account;
  • Selecting the location of a reported issue;
  • Attaching a photograph;
  • Requesting email or text-message updates; and
  • Reviewing previously submitted reports and their status.

The Service is intended for issues that fall within UDOT’s responsibilities. Reports may be referred to another government entity when appropriate.

2. Not an Emergency Service

UDOT SR is not an emergency-reporting service and should not be used when an immediate response is required.

For emergencies, crashes, threats to life or safety, crimes in progress, or immediate roadway dangers, call 911 or contact the appropriate emergency authority.

Submitting information through UDOT SR does not guarantee an immediate response.

3. Eligibility

The Service is available to members of the general public in the United States. If you are a minor, you should use the Service with the assistance of a parent or legal guardian when appropriate.

You must be legally permitted to use the Service and must comply with these Terms and all applicable laws.

4. Guest Use and Registered Accounts

You may submit certain service requests without creating an account. Registered accounts may provide additional features, such as access to report history.

When creating or using an account, you agree to:

  • Provide accurate and current information;
  • Maintain the confidentiality of your credentials;
  • Use only an account that belongs to you;
  • Promptly update inaccurate account information; and
  • Notify NIKALogic if you believe your account has been accessed without authorization.

You are responsible for activity conducted through your account, except to the extent caused by NIKALogic’s failure to use reasonable security measures.

5. Service Requests

When submitting a service request, you agree to provide information that is accurate to the best of your knowledge.

You should include only information reasonably necessary to identify and describe the roadway issue. Do not submit Social Security numbers, financial account details, medical records, confidential credentials, or unnecessary personal information concerning another person.

Submission of a report does not create a contractual obligation for NIKALogic, UDOT, or the State of Utah to inspect, repair, investigate, or resolve the reported condition.

UDOT determines how reports are categorized, prioritized, assigned, investigated, and resolved. A report may be transferred, combined with another report, closed, or determined to be outside UDOT’s jurisdiction.

6. Photographs and Other Submitted Content

You retain any ownership rights you hold in descriptions, photographs, and other material you submit through the Service.

By submitting content, you grant NIKALogic, UDOT, and their authorized service providers a nonexclusive, royalty-free license to host, store, copy, process, reproduce, transmit, redact, disclose, and otherwise use that content as reasonably necessary to:

  • Operate the Service;
  • Review and respond to the reported issue;
  • Route the report to the appropriate government entity;
  • Maintain government records;
  • Comply with applicable law; and
  • Perform related governmental functions.

You represent that you have the right to submit the content and that its submission and permitted use do not violate another person’s rights.

7. Acceptable Use

You may not use the Service to:

  • Submit knowingly false, fraudulent, or misleading reports;
  • Impersonate another person or misrepresent your identity;
  • Harass, threaten, defame, or unlawfully target another person;
  • Upload unlawful, malicious, obscene, or infringing content;
  • Introduce malware or harmful code;
  • Attempt to bypass authentication or security controls;
  • Obtain unauthorized access to accounts, systems, or data;
  • Interfere with the operation of the Service;
  • Use automated methods to submit reports or access the Service without authorization;
  • Reverse engineer the Service except where applicable law expressly permits it; or
  • Use the Service for any unlawful purpose.

NIKALogic may restrict or suspend access when reasonably necessary to protect the Service, UDOT, users, or the public.

8. Email and Text Messages

You may choose to receive verification codes and service-request updates by email or text message. These communications are transactional and relate to your account or submitted reports.

Message frequency varies. Message and data rates may apply. You may opt out of text messages using the instructions included in the message, such as by replying STOP where supported.

Consent to receive optional status notifications is not required to submit a service request.

9. Maps and Location Information

Maps, address results, geocoding, device-location information, and geographic boundaries may be provided by third parties or generated from data that is incomplete, delayed, or inaccurate.

You are responsible for reviewing the selected report location before submitting a request. NIKALogic and UDOT do not guarantee the accuracy of map, address, GPS, or location information.

10. No Guarantee of Action or Status

NIKALogic transmits service requests for review by UDOT. Neither NIKALogic nor UDOT guarantees:

  • That every submission will be accepted;
  • That a reported issue will be inspected, repaired, or resolved;
  • A particular response or completion time;
  • A particular result or priority;
  • That status information will always be current; or
  • That the Service will always be available.

UDOT SR is not a method for submitting a legal claim, administrative appeal, public-records request, court filing, or other formal or time-sensitive legal notice to NIKALogic, UDOT, or the State of Utah.

11. Government Records

Reports are not displayed publicly through the Service. Information delivered to UDOT may nevertheless constitute a government record and may be retained, disclosed, withheld, or redacted as required or permitted by applicable law.

Do not submit information that is unnecessary to describe the roadway issue.

12. Third-Party Services

The Service may rely on services provided by Google, Firebase, Apple, Twilio, mapping providers, telecommunications carriers, and other service providers. Your use of those services may also be subject to the provider’s terms and privacy policies.

NIKALogic does not control and is not responsible for third-party services, telecommunications networks, app stores, or external websites.

13. Intellectual Property

Except for content submitted by users and materials owned by UDOT or third parties, the Service and its software, design, text, graphics, and other components are owned by or licensed to NIKALogic and are protected by applicable intellectual-property laws.

UDOT names, marks, logos, and branding belong to UDOT or the State of Utah. These Terms do not grant permission to use those marks.

You may use the Service only for its intended public-service purposes.

14. Service Changes and Availability

NIKALogic or UDOT may modify, suspend, restrict, or discontinue all or part of the Service at any time. Features and eligibility requirements may change as the Service develops.

We may perform maintenance or take emergency action that temporarily makes the Service unavailable. We are not responsible for reports that cannot be submitted because of network, device, third-party, or service interruptions.

15. Disclaimer of Warranties

To the fullest extent permitted by law, the Service is provided “as is” and “as available.”

NIKALogic disclaims warranties of merchantability, fitness for a particular purpose, noninfringement, accuracy, availability, and uninterrupted or error-free operation to the extent such warranties may legally be disclaimed.

Nothing in these Terms creates a warranty, contractual promise, or legally enforceable commitment by UDOT or the State of Utah to take a particular action in response to a report.

16. Limitation of Liability

To the fullest extent permitted by law, NIKALogic, UDOT, the State of Utah, and their officers, employees, contractors, and service providers will not be liable for indirect, incidental, consequential, special, exemplary, or punitive damages arising from or related to:

  • Use of or inability to use the Service;
  • Delayed, lost, incomplete, or unsuccessful submissions;
  • Inaccurate map, address, location, or status information;
  • Actions taken or not taken in response to a report;
  • Unauthorized access by third parties; or
  • Third-party services or telecommunications networks.

Nothing in these Terms waives any governmental or sovereign immunity, statutory protection, limitation, defense, or procedural requirement available to UDOT or the State of Utah under applicable law.

Some jurisdictions do not permit certain limitations of liability, so portions of this section may not apply to you.

17. Privacy

The collection and use of information through the Service are described in the UDOT SR Privacy Policy:

https://udot-public.nikalogic.com/privacy-policy.html

18. Governing Law

These Terms are governed by the laws of the State of Utah, without regard to conflict-of-law principles.

Any legal action concerning the Service must be brought in a state or federal court of competent jurisdiction located in Utah, subject to all applicable governmental-immunity, notice, venue, and procedural requirements.

19. Changes to These Terms

We may update these Terms to reflect changes to the Service, our practices, or applicable law. Updated Terms will be posted with a revised effective date.

Your continued use of the Service after updated Terms take effect constitutes acceptance of the revised Terms.
